for Undergraduate Mathematics Classes

Form for Testing a Module

Instructors can register for testing a module by filling out the following blanks. Instructors selected to test one or more of the MPE sustainability modules by October 19, 2012 will be eligible for up to $600 reimbursement of their travel costs to the 2013 Joint Mathematics Meeting (JMM) in January 2013 being held in San Diego, CA. If selected, instructors will be sent a pdf copy of the module, instructions on the testing process, an evaluation form to be filled out after the testing is completed, and a link to the online DIMACS reimbursement form. Once the completed evaluation form has been returned to DIMACS, and after returning from the JMM, testing instructors should complete the travel reimbursement form and mail it to DIMACS along with original receipts.

Selection of testing instructors will continue while funding is available.
