1. Two page Application Form
2. Statement of Understanding: signed by applicant and school principal.
3. Graduate Credit Requirements: signed by applicant.
Required: Participants must have e-mail access from May 2004 through August 2005.

Although these items may be mailed separately, only complete application files will be reviewed.
(Early Acceptance Deadline: January 16, 2004).
Applicants to be accepted on a first-come, first-served basis.
Applications will be accepted until all slots are filled
